We offer flexible membership options to suit different needs:
- Single: $29.16/month or $350/year
- Dual: $54.16/month or $650/year
- Family: $81.00/month or $972/year (includes up to 4 children under 18)

- 50% off Sealants and Whitening
- 20% off Crowns, Veneers, Deep Cleanings, and Partials
- 15% off Fillings and Root Canals

Plan Details and Exclusions
Important Guidelines
The Cottage Grove Dental Membership Plan is a discount plan, not dental insurance.
It cannot be used with other dental insurance plans or for services covered under worker’s compensation, referrals to specialists, or hospitalization charges.
Accepted Payments
We gladly accept personal checks, most credit and debit cards, cash, FSA cards, CareCreditTM, and LendingPointTM.
Monthly plans are financed through LendingPointTM. The cost of the plan is non-refundable, and payment is due at the time of service.
